Talent Release Form

I have read and agreed to the Talent Release Agreement.
I , as a volunteer or paid, independent contractor, hereby assign and grant Media Quest Inc. and
its Client, absolute and irrevocable right and
permission to use and publish the reproduction, sale, written extraction, copyright, exhibit,
broadcast, and electronic storage of the following: Photographs, Films, Video tapes, Electronic representation, and/or sound recordings.
I am granting permission to also copyright these same products in the name of Media Quest
Inc., and its client. I am granting permission for Media Quest Inc. and its client to use, re-use,
publish, and re-publish the same (in whole or in part), individually or in conjunction with other
video images, in any medium and for any purpose, as designated by Media Quest and it's client
including illustration, promotion, and advertising trade.
This authorization and release shall also ensure to the benefit of the legal representatives,
licensees and assigns of Media Quest Inc, as well as, the client(s) for whom Media Quest Inc.
took the video images and/or audio recordings.
